Following a number of newborn infant deaths at the neonatal ward of the Countess of Chester Hospital in northwest England in 2015 and 2016, the nurse was detained. Her last victims were two triplet boys who were known in court as babies O and P.

Letby refuted the prosecution's other allegation that she had a hidden connection with a married doctor who worked at the Countess of Chester hospital. However, the prosecution presented evidence to the court showing that the two frequently messaged, exchanging love heart emojis, and met outside of the workplace.
They noted that it was crucial because the doctor was one of the first people Letby called when a baby's health unexpectedly declined. The doctor, with whom she may have been having an affair and whose name was kept a secret for legal reasons, was the target of these activities, according to the prosecution.

Letby testified to the jury that although though she was a band 5 nurse and qualified to care for critically ill newborns in the neonatal intensive care unit, she did not find her work fascinating when she was assigned to care for infants who did not require extensive medical attention.

Investigators were able to get a glimpse into Nurse Letby's mental condition thanks to a few Post-it notes. Investigators discovered Post-it notes with Lucy's writing on them when she was arrested in 2018 and they said, "I killed them on purpose because I'm not good enough to care for them," and "I AM EVIL I DID THIS."
She added, "I will never marry or have children. I'll never understand what it's like to be a family man.
Letby claims that these notes were created following her work suspension while the strange deaths were being investigated.
She was charged with intentionally injuring the infants in a number of ways, including by injecting air into their bloodstreams and giving them air or milk through nasogastric tubes. She was also charged with messing with breathing tubes, introducing insulin to intravenous feeding, and poisoning newborns.
Soon after the verdicts, the British government established an impartial investigation that will go deeper into the facts around what transpired at the hospital, including how staff complaints were handled.
According to Health Secretary Steve Barclay, the investigation will work to make sure the parents and families affected receive the information they require. "I am determined that their voices are heard, and they are involved in shaping the scope of the inquiry should they wish to do so."
Senior physician at the Countess of Chester Hospital told the BBC that despite his repeated attempts to alert hospital administrators about Letby, they did not look into the claims.
Lead neonatologist Dr. Stephen Brearley claimed the hospital attempted to intimidate physicians who voiced complaints about Letby and put off notifying the police.
